Can it scale-up, has it been tested with a large volume of data ?
Yes, we artificially created 1024 patients in Obsidian MEDILIG, each with their own medical record. As shown in the screen capture video, under the folder People/Patient
where all patient records are stored, there are a total of 7,178 files and 2,047 folders. All functions of Obsidian MEDILIG operate without issues. More demanding actions such as:
- searching by full name across all names
- alphabetical sorting
- scrolling through a table of 200 names
- viewing scheduled appointments for the week and
- opening a random patient record
have minimal and expected delays given this volume of data.

Under which operating system does Obsidian MEDILIG run?
In theory, it runs on all platforms since it is built on top of Obsidian. In practice, however, it has only been tested on Windows environment. There may be issues on other operating systems due to differences in file paths, which can affect the JavaScript code used in the Obsidian MEDILIG templates.
Does Obsidian MEDILIG support multiple users and concurrent tasks ?
Obsidian can be used by multiple users by creating a shared folder on a local network, installing Obsidian on each computer, placing the Obsidian MEDILIG application in the shared folder, and opening the Obsidian Vault of MEDILIG from each user’s computer. We tested this setup with two users, who were able to edit the same patient's file, at the same record location within the file.
However, an Obsidian Vault is not designed for real-time multi-user collaboration. It is better suited for personal use or asynchronous collaboration with coordination among users.
Can I sync, keep my data consistent, across multiple devices ?
Absolutely, you can sync the data and files of Obsidian across multiple devices. There are several ways to do this:
- Obsidian Sync: This is Obsidian’s official sync service. It is a paid service that offers end-to-end encryption.
- Other services: You can also use third-party solutions like OneDrive, Google Drive, Syncthing, iCloud, or Dropbox by storing the Obsidian MEDILIG vault in your cloud-synced folder
This allows you to access and update the same vault from multiple devices, provided proper sync coordination is maintained.
Do I have to pay for Obsidian MEDILIG distribution
Obsidian MEDILIG is an open source (GNU-AGPL) application that is distributed for free for personal use. However, if you find it useful and/or are using Obsidian for work in an organization, we encourage you to make a donation of 9€ or more. This we help us gradually cover the costs of code development and testing for two versions of the software in Greek and English, user manual creation also in Greek and English, and packaging/distribution to end users. Additionally, your contributions will allow us to release a public version of the project on GitHub.
You are not required to pay for a commercial license for Obsidian, the editor and environment, host application, that MEDILIG is running on top of it (see pricing).
Do you store, access, or process user data?
Obsidian MEDILIG is a private and secure application. You can use Obsidian MEDILIG without sharing any personal information, and your data is stored locally on your device, making it inaccessible to outside. We do not collect telemetry data, and we never sell user data.
What are the support options you provide for the application
Any support services are provided by agreement with Dr. Athanassios I. Hatzis (athanhat@gmail.com), scientific associate and consultant at HEALIS.
How can I take a backup ?
On the vertical ribbon on the left side of Obsidian, there is a Run Local Backup icon Fig. 1. When you click it, a compressed file containing all the files from the Obsidian MEDILIG vault will be created.
