Naming and Coding
The file in which the patient’s medical history is recorded has the code name mhXYZ.medhist
, where XYZ is the patient’s numerical identifier. For example, mh102.medhist is the medical history for GEORGIOS_ANTONIOU_6998765432.
Content of the Medical History
The medical history is divided into the following sections, see use of internal links, in the table of contents at the beginning of the file. By clicking the appropriate link, we are taken directly to the exact section of the patient’s medical history.
Embedding/Transclusion

Thanks to the embedding feature in Obsidian, we can, for example, embed the patient’s family history right at this point in the document.
This allows you to reuse and display content from another note without duplicating it, keeping records consistent and easier to maintain.
Folding Sections
Obsidian also supports text folding and unfolding (folding). We have placed two buttons for this functionality above the banner image (see Fig. 1):
- Fold all headings and lists – collapses all headings and lists.
- Unfold all headings and lists – expands all headings and lists.
This feature helps keep long medical notes or patient histories organized and makes navigation much easier. This allows clinicians to quickly navigate long medical histories while focusing on the relevant sections.

Front-Matter Data
The information entered in the medical history form fields is automatically captured in the front-matter fields (Fig. 2). You can access these fields from the right-hand panel of the application by clicking the 🛈 icon. This allows for structured metadata management and easy reference across the patient’s record.
