Naming and Coding

The file in which the patient’s medical history is recorded has the code name mhXYZ.medhist, where XYZ is the patient’s numerical identifier. For example, mh102.medhist is the medical history for GEORGIOS_ANTONIOU_6998765432.

Content of the Medical History

The medical history is divided into the following sections, see use of internal links, in the table of contents at the beginning of the file. By clicking the appropriate link, we are taken directly to the exact section of the patient’s medical history.

Embedding/Transclusion

Thanks to the embedding feature in Obsidian, we can, for example, embed the patient’s family history right at this point in the document.

This allows you to reuse and display content from another note without duplicating it, keeping records consistent and easier to maintain.

Family History

Parents:

Father: 72 years old, retired. History of hypertension and cardiovascular disease. Underwent angioplasty 5 years ago.

Mother: 70 years old, retired. History of type 2 diabetes and osteoarthritis. Underwent hip replacement surgery 3 years ago.

Siblings:

Brother: 45 years old, lawyer. History of childhood asthma, well managed with medication. Married with two children.

Sister: 40 years old, teacher. History of migraines, managed conservatively. Married with three children.

Children:

Son: 15 years old, student. History of pollen and certain food allergies, managed with antihistamines.

Daughter: 12 years old, student. Healthy, no significant medical issues to date.

Grandparents:

Paternal grandfather: Died at 78 from a heart attack. History of hypertension and hypercholesterolemia.

Paternal grandmother: Died at 80 from a stroke. History of type 2 diabetes.

Maternal grandfather: Died at 75 from lung cancer. Long-term smoker.

Maternal grandmother: Died at 82 from natural causes. History of osteoporosis and hypertension.

The patient’s family history indicates a predisposition to cardiovascular disease, diabetes, and hypertension, with additional history of lung cancer, asthma, and allergies.

Folding Sections

Obsidian also supports text folding and unfolding (folding). We have placed two buttons for this functionality above the banner image (see Fig. 1):

  • Fold all headings and lists – collapses all headings and lists.
  • Unfold all headings and lists – expands all headings and lists.
    This feature helps keep long medical notes or patient histories organized and makes navigation much easier. This allows clinicians to quickly navigate long medical histories while focusing on the relevant sections.

Front-Matter Data

The information entered in the medical history form fields is automatically captured in the front-matter fields (Fig. 2). You can access these fields from the right-hand panel of the application by clicking the 🛈 icon. This allows for structured metadata management and easy reference across the patient’s record. These fields can be used also for filtering, querying,
